HELP WITH THE FORM

In the first box select whether you'd like a pram or a stork to be engraved on the front of the jade block.

 

In the second box fill in the baby's first and last name.

 

In the third box fill in the baby's date of birth.

 

In the fourth box enter a message for the lid of the gift box.  This will be engraved onto a presentation plaque.  You are allowed a maximum of 50 characters for the message.

 

In the fifth box you can enter the baby's weight at birth and in the sixth box you can enter the time of birth, however, please note these last two fields are entirely optional.
